If you have only had one Niqah with your wife, and you decide to issue her with a talak and she has observed and exhausted the Iddah period then you can remarry her with a new Niqah and new witnesses along with a new Mahar.

However if you have married your wife 3 times and divorced her as many times then she has naturally married again and moved on, then for some reason there has been a breakdown with her new marriage and as a consequence been divorced then she is free to remarry you, but if this is constructed or planned, then it becomes Haram.
